The exercise ends with a clean decision log and every played objective marked successful. Weeks later, a worker is admitted to hospital after a serious incident. The response cell now has to contain the hazard, qualify conflicting reports, preserve evidence, contact family and decide which authority receives what. The legal clock is already moving.

Your exercise result may still be sound. The claim made from it is too broad. A controlled test shows what selected people did against selected criteria under selected conditions. Live reporting adds start events, parallel work and evidence duties that may never have entered play.

That gap matters because a good after-action report can lend false certainty. The report says the team notified the regulator. It may say nothing about when the clock began, whether a controller supplied the regulator’s details, or who logged the decision while the lead managed the event.

Write the conclusion before you write the scenario. Evaluation becomes impressionistic when an objective such as “validate notification” reaches the evaluator without a task, criterion or evidence route. A polished inject cannot repair an undefined measure.

For every objective, name the function, observable tasks, expected criterion, data sources, evaluator and decision rule before play. HSEEP 2020 Exercise Evaluation Guides map results to objectives, capability targets and critical tasks. Its capability-target model uses three parts: critical task, impact and timeframe.

Record where the criterion came from. A legal limit, approved procedure, service level and sponsor-approved exercise threshold support different claims. An invented exploration threshold can reveal a delay. It cannot become a binding standard because participants happened to beat it.

Define aggregation at the same time. State whether every critical task must succeed, whether one safety-critical miss overrides the rest and how unavailable evidence will be handled. Do not average elapsed time, form completeness and evaluator judgment into one readiness score.

Keep not observed as a real result. Pass-or-fail reporting fails when the scenario never reaches an objective. The evaluator then penalises participants for missing play or gives the sponsor an unsupported success. Both choices hide a defect in the test.

Use four results after the observation is written: performed, partially performed, not performed and not observed. Link each to the stimulus, action, available information, evidence, effect and criterion. If the objective received no adequate stimulus, or the evaluator could not see it, preserve not observed and redesign the test.

The exercise itself needs a separate finding. AIDR 2016 separates exercise outcomes from exercise management. That two-part distinction lets you record competent performance under a weak test without treating the weak test as assurance.

Put the test conditions beside the finding. A tabletop feels demanding because decisions arrive quickly. Physical systems, absent agencies and routine work may still be represented by controllers. Success under that artificial control says less about a live cell than the unqualified sentence suggests.

HSEEP 2020 recognises four discussion-based and three operations-based exercise types. NIST SP 800-84 gives two to eight hours as a typical tabletop duration. Neither label settles fidelity. State who played, who was simulated, which functions and shifts were excluded, which systems were real, what participants knew beforehand, and where coaching changed events.

Record every material controller intervention. If a controller supplies the missing regulator number, the downstream call may be demonstrated while the upstream retrieval gap remains open. Split those conclusions. Also mark evaluator coverage by critical task rather than by room, because an occupied observer can still miss the decisive action.

Start the clock on the reportable event. Internal targets drift into legal claims when the exercise uses one unlabeled “notification time”. That measure conceals both the governing event and the difference between sending, delivery, acknowledgement and engagement.

Write each jurisdictional branch separately. In the US federal OSHA regime, place work-related fatality beside the eight-hour limit. Place in-patient hospitalization, amputation or eye loss beside the 24-hour limit. Start exercise timing when the employer learns that the specified outcome has occurred, then test which facts establish the correct branch.

Great Britain uses another structure. RIDDOR requires notice without delay for a work-related death, specified injury or dangerous occurrence, with the report received within 10 days of the incident. Over-seven-day incapacity is reportable within 15 days. Australian model-WHS guidance starts immediate notification when the duty holder becomes aware of a notifiable event and also brings site preservation into the response.

Keep your internal acknowledgement target in its own field. Set it from time to irreversible harm and the time a substitute needs to act. A legal outer limit does not tell you how quickly a technical lead must engage before evidence disappears.

Size the tree from work that must start together. A fixed call list can look fast in a tabletop because one controller plays several agencies and nobody is handling live family data. During the incident, serial calls make safety, legal, technical, evidence and family work wait behind each other.

Map concurrent decisions, then give every critical branch a primary, deputy, alternate channel and advance-on-no-ack rule. Test send, delivery, acknowledgement and engaged states separately. The UK HSE’s internal contact log has nine fields covering initial notice, recipients, updates and completion. Its 12-month retention rule belongs to that UK internal record, not to every company.

Use the same workload test for the cell. ISO 22320:2018 covers roles, tasks, resources and cooperation across incident types and scales, yet its published scope gives no headcount. US OSHA’s nonmandatory ratio of one fire-evacuation warden per 20 employees is a coverage illustration, not a response-cell formula.

Add capacity when decisions queue, reports arrive faster than they can be qualified, several agencies need liaison or restricted information creates a separate stream. Protect tactical ownership, technical work and contemporaneous logging. UK Home Office guidance identifies the conflict between commanding and taking full notes, and expects decisions to remain intelligible up to seven years later.

Exercise the information state and its custody. A team can make the right call from a controller’s perfect summary and still fail with live reports. Repetition then turns an unverified claim into the apparent fact on which several branches act.

Mark each material entry observed, reported or confirmed. Preserve disproved entries through a linked correction. Keep sensitivity separate from confidence. Use one timestamp standard and record event time where it differs, because ISO 15489-1:2016 covers creation, capture, metadata, responsibilities and records controls across forms.

Make the selected cell work the evidence, not merely discuss it. Inject conflicting source reports while the lead decides, the loggist records rationale and options, and the legal branch identifies its clock. The result can then support a bounded claim about information load and custody under those stated conditions.

Close the finding after demonstrated change. Improvement plans fail when “complete” means a procedure was edited or training was booked. Those actions may remove a cause. They provide no evidence that the task now works under pressure.

Keep administration measures separate. US EPA’s 2019 rulemaking retained a 90-day deadline for covered tabletop and field-exercise reports. GAO recorded a Coast Guard reporting limit reduced from 60 days to 21 days, then reviewed 26 reports that contained objectives and lessons. Better, faster reports improved usability. They did not establish operational effect.

Name the later check when the action is assigned. Recreate the original condition in a targeted retest, or use an inspection that can demonstrate the changed capability. Record findings that cannot yet be recreated and those still lacking effect evidence. Under US 10 CFR Part 50 Appendix E, an unsatisfactory biennial test can require remedial exercises. The assurance stays tied to the missed performance opportunity.

Your final conclusion should be narrow enough to survive the live clock. State the task demonstrated, criterion met, people and systems involved, artificial support, missing coverage and remaining retest. That sentence may sound less impressive than “ready”. It tells the incident lead exactly what can be relied upon.
